The topic that we are presenting is the process of change as it relates to creating “A New Chart Order” in an acute care hospital. This is a presentation about a real life change that occurred in a hospital in Alberta. Background information will provide readers with an understanding of the problem with the old chart order, the reason for change and the vision for the desired state. Potential enablers and barriers of acceptance of a new chart order are discussed along with strategies to promote acceptance and sustain implementation of the new chart order. Lastly, we will present the actual strategy used by the hospital to implement the change.
The objectives for our presentation are:
1) To demonstrate a successful organizational change;
2) To identify the enablers and barriers to organizational change;
3) To identify key elements of successful change; and
4) To provide resources and potential team building activities to assist in organizational change.
